Registration & Enrollment Procedures

Requirement for Admission of NEW STUDENTS
  • Child aged 2 to 6 years old can be admitted to the Pre-School level, and child aged 6 to 12 years old and above, (depending on the results of the test and interview and entrance exam).
  • The child will be placed according to the favorable results of the testing/interview.
  • Submission of the following forms is required:
    • Accomplished Registration Card/Application Form
    • Valid Pre-Enrollment Agreement
    • Photocopy of Birth Certificate
    • ID pictures (blue background [1x1=2pcs.] [2x2=2pcs.])
    • Certificate of Completion/Form 138 from the former school (transferees & Grade 1-6, 1st Year High School ID, 4th Year High School)
    • Certification of good moral conduct from the Principal of the former school from Grade 1 to Grade 6, 1st Year - 4th Year High School
    • 2 pcs. Long brown envelope
    • For Foreign Students, secure Special Student Permit from the Bureau of Immigration
Requirement for Re-Admission of OLD STUDENTS
  • OLD STUDENTS are always given priority to re-enroll at MdeZ as such, reservation forms are issued in the second semester for students to secure a slot for the following school year.
  • Reservation forms are to be filled up immediately and requirements contained therein should be complied with to confirm the students reservation WITHIN THE PERIOD stated in the form.  Failure to comply with any of the requirements for reservation automatically cancels priority given to students.
  • OLD STUDENTS who fail to return the reservation form or comply with any of the requirements for reservation will be placed in the waiting list along with new students.
  • RESERVATION FEE of Php500 must be paid to assure a slot for your child for the year and level being applied for.  Since enrollment is on a very limited basis only.  RESERVATION FEE is non-refundable.
